Two agencies to get state grants

Two area agencies are marked to receive $100,000 state Department of Health grants aimed at moving nursing home residents into community-based care.
Targeted for the funds are the Center for Disability Rights Inc. and Western New York Independent Living Inc.
The money comes under Money Follows the Person, a demonstration program to provide nursing home residents with information about community-based care options. The initiative is part of a broader health department effort to shift care away from institutional settings, state Commissioner of Health Richard Daines M.D. said.
The local agencies are two of nine non-profits across the state marked to get a total of $1.9 million.
The Rochester-based Center for Disability Rights serves Monroe, Ontario, Wayne, Livingston, Seneca and Yates counties. Western New York Independent Living is based in Buffalo and serves the counties of Orleans, Wyoming, Erie, Niagara and Chautauqua.
