Responsible for providing professional nursing supervision to the agency's home care consumer as prescribed by the physician and in compliance with the state's Registered Nurse Practice Act, any applicable licensure/certification requirement and agency policies and procedures.The Registered Nurse is responsible for tracking and conducting the required supervision visits with assigned consumers and PCAs and providing the appropriate level of supervision for PCAs when taking on a new consumer assignment. Quality of Care: 1.Uses professional nursing skills of assessment, planning, implementation and evaluation in working with client/family.
2.Identifies present and potential needs and resources in relation to the health of client [family. Revises written care plan based on assessed changed needs and goals set mutually with client/family and nurse.
3.Evaluates care plan for progress towards goals and revises as needed.
Demonstrates awareness of and adherence to agency policies and procedures ensuring safe and skilled nursing care.
Documentation: 1.Completes all agency records and reports according to agency policy and procedure. Accurately documents client care observations, interventions and evaluations on day services are rendered.
2.Documentation is legible.
3.Submits documentation in a timely manner.
1.Appearance is professional and complies with agency dress code.
2.Assignments are fulfilled with reliability and punctuality.
3.Maintains a cooperative manner towards client/family and all members of the health care team.
4.Listens and responds to client's needs in a sensitive and timely manner either through own initiative or by referral to appropriate person(s).
1.Graduate of an accredited School of Nursing.
2.Valid and Current New York State Registered Nurse License and Registration.
3.Minimum of one (1) year of Homecare.
4.Excellent verbal and written communication skills.
5.Ability to function with minimal direct supervision, to make decisions, to problem solve, and to handle emergencies.
6.Ability to relate to a diverse and multi-cultural population.
7.Possess a valid driver's license and have a car available for use.
