Manager of Service Coordination



Employment Type: Full-Time
Summary:
To provide oversight to the NHTD/TBI service coordination program under the TBI, NHTD, and OMRDD waiver. The manager of service coordination is expected to meet any potential participants prior to the completion of the initial service plan developed by a service coordinator under their supervisor, have supervisory meetings with staff on at least a bi-weekly basis, and review and sign off on all service plans.
Duties:
As a representative of CDR be committed in all you do to support, perpetuate and model the CDR Mission.

Monitor and maintain Spreadsheet of all plan and PRI and Screen expiration dates. Assure that plans are not allowed to expire.

Move plans appropriately through the team meeting, corrections and NOD processes.

Serve as the RRDC Primary Contact for TBI, NHTD and OMRDD Waivers.

Hiring of new staff.

Screening, interviewing, hiring, training disciplining and supervising staff.

Review TBI/NHTD/OMRDD related materials with staff upon hire and provide on-going staff training.

Review agency policies and procedures with staff.

Conduct performance Evaluations with staff.


The manager of service coordination will work to provide quality services and customer satisfaction. Conduct spot checks.

Recommend areas of staff improvement and potential training opportunities.

To provide staff training as necessary.

Provide feedback to staff on job performance on a regular basis.

The Manager is responsible for meeting the budget expectations in each program that they supervise.

On-call after hours rotation with other Managers in the Waiver department.

Ensure staff is working on the goals and are knowledgeable about what each goal entails.

Ensure schedules are developed based on the number of approved hours per week and the number of hours per unit.

Follow the progressive discipline system for all employees that have any performance concerns to ensure that quality services are provided to all consumers of CDR.
Skills:
Ability to multi task.

Good communication skills and listening skills.

Handling of TBI and NHTD waivers.

Training, supervising, interviewing and hiring skills.
Qualifications:
Must have either type A or type B qualifications to be considered (Type A)

Must have one of the following educations

Master or Doctorate of Psychology

Master of Social Work

Registered Physical Therapist

Professional Registered Nurse

Certified Special Education Teacher

Certified Rehabilitation Counselor

Licensed speech-language Pathologist

Registered Occupational Therapist

Must have a minimum of one (1) year of experience providing service coordination and information, linkages and referrals regarding community-based services for individuals with disabilities

(Type B) Must have the following education

Bachelors degree in a human services related field (Example: Social Work, Psychology, Human Services.

Must have a minimum of two (2) year of experience providing service coordination and information, linkages and referrals regarding community-based services for individuals with disabilities.

This position requires travel. Must have reliable transportation.
